Step 1 - The Catalog

Complete the order. Print accurate and complete information. Do NOT pay the $2 processing fee. It does NOT apply to our school. (Do NOT mark-up the form, just ignore the fee).
Include seller and organization name plus email address to receive confirmation.
Enclose white/yellow copies with payment, and keep pink copy.
Allow 10-14 weeks for delivery on magazines.
Gift items will arrive 2 weeks of program completion to your home.
If paying by check, make check payable to: SIXES ELEMENTARY PTA.

Step 2 - The Postcard Booklet

Help your child fill out each postcard with names and addresses of family and friends you don’t see regularly.
Do not tear postcards out. These booklets will be collected and sent to addresses with a list of items to support the fundraiser. These names will only be used by QSP.
Your child will receive a FREE Prize for the completed booklet.
